Embrace Healthy Aging

Healthy aging is about more than growing older. It is about maintaining independence, connection, and well-being at every stage of life. The Capital Area Public Health Network (CAPHN) works with community partners to strengthen environments and systems that help older adults remain active, engaged, and supported in their communities.

Focus Areas

Healthy aging work in the Capital Area is driven by collaboration, shared data, and coordinated action across healthcare, community services, and municipal systems. Together, partners focus on:
Chronic Disease Prevention & Management – Increasing participation in local programs and services 
Falls Prevention – Reducing injury, disability, and death caused by older adult falls 
Caregiver Support – Expanding capacity and resources for family caregivers 
Social Connection – Promoting engagement and reducing isolation

The Capital Area Healthy Aging Coalition brings partners together to strengthen healthy aging efforts across the region, including chronic disease prevention, falls prevention, caregiver support, and opportunities that reduce social isolation among older adults.
